Cancer cells are classified in two groups depending on their origin: primary and metastatic. Moreover, cancers are also addressed in categories and sub-categories regarding the organs and cells they originate from. &nbsp;&nbsp;Final diagnosis of the cancer is made with pathological examination of biopsy specimens after general characteristics and nature of the cancer are revealed out by imaging modalities. The most appropriate treatment option(s) is /are determined after the stage of cancer is identified on the final step.Staging of the cancer: Staging is somewhat determining the level of progression (metastasis) and aggression. Cancer stages are expressed with Roman numbers (from I to IV). Stage 0 or 1 implies that the cancer is yet to spread from its original location and the cancers in this stage usually respond to treatment well. &nbsp;&nbsp;In other words, the chance of cure is higher in low stage cancers. On the other hand, stage IV cancers represent tumors which had already spread (metastasized) to distant tissues and organs.Medical oncology uses the option of medication therapy for treatment of cancer. These include chemotherapy, hormone therapy, targeted therapies and biologic therapy.Chemotherapy: Chemotherapy is the generic name of drugs which are used to terminate and/or limit the division and growth of cells which divide or grow in an uncontrolled manner due to genetic or environmental factors. While chemotherapy can be used alone, it can also be administered before and after a surgical treatment or in combination with radiotherapy, whenever required (combination treatment). &nbsp;&nbsp;Anti-cancer drugs or chemotherapeutic agents can be administered via oral (tablet, capsule or solution), intravenous, intramuscular, subcutaneous routes or directly into the tumor/lesion (intralesional).The effectiveness of chemotherapy drugs is based on the fact that cancer cells multiply in an uncontrolled manner. In other words, chemotherapy drugs have higher effect on cells that divide and grow in an uncontrolled manner in comparison to healthy cells.&nbsp; However, chemotherapy drugs also affect the healthy tissues and cells of the body.The chemotherapy itself does not cause pain for the patient.Side effects of chemotherapy drugs include a wide spectrum of complaints depending on the anti-cancer drug and overall health of the patient. Side effects of chemotherapy drugs originate from effects of the treatment on healthy cells of the body.Hormone therapy: Certain cancers, such as prostate and breast cancers, are susceptible to hormones. Certain hormones play a role in development of these cancers and they are, therefore, responsible for their uncontrolled division and growth. The purpose of this treatment is to suppress the level of hormone which leads to abnormal division of cancer cells in the body in order to regress and destroy the cancer.Similar to chemotherapy, the side effect profile depends on the drug and certain patient factors (gender, overall health etc.).Biological therapies: Biological treatment is used to slow down and/or stop division and growth of the cancer cells by activating the native factors that form the defense mechanism of the body (antibodies, interferon etc.). This type of treatment is still intensely studied in clinical trials and certain agents are still in experimental stage.Targeted therapies: This is one of the most important innovations that the modern medicine gained in treatment of cancers. The treatment mechanism is based on killing the cancer cells by binding to certain proteins produced by them. Medical Oncology

Radiation Oncology is a discipline of medicine that uses ionizing radiation to destroy and/or kill cancerous cells. This treatment modality is called radiotherapy or radiation therapy. Another treatment method is radiation surgery (or radiosurgery) which has been introduced into use in the field of Radiation Oncology thanks to the advancements in physics and computer technologies.The ionizing radiation used for treatment damages the genetic structure and DNA of the cancerous cells and prevent the abnormal and uncontrolled cell division and growth. Thus, the tumor shrinks and/or the cancerous cells are eliminated.Beams used during treatment are produced and delivered to the cancer focus by the cutting-edge machines. These machines are called Linear Accelerator. This method is called external radiotherapy and it represents the most common radiotherapy technique. The patient feels no pain during external radiotherapy.Another method is internal radiotherapy (or brachytherapy). The radioactive material is placed inside or nearby the tumor temporarily or permanently. Internal radiotherapy does not cause pain.The radiotherapy is planned and performed by radiation oncologists and physics engineers. In the past, intact tissues were also exposed to the devastating effects of radiation along with the tumor tissue. In addition, long-term exposure was inevitable when it was necessary to deliver high-dose of radiation to the tumor.Today, the beams used for treatment are focused on the tumor with very high accuracy thanks to advancements in physics and computer technologies, and thus, the effects on the intact tissues can be minimized. Furthermore, the beams can be delivered with very high accuracy (below one millimeter), as breathing-related movements of organs, where the tumor originates from, are taken into consideration.Encircling the cancer in all directions: Conformal TherapyAnother striking development in the radiation therapy is the integration of Linear Accelerators, the devices that generate and emit the ionizing radiation, with imaging devices. In this way, a three-dimensional image of the tumor is obtained and the radiation beams are shaped to conform to the target volume (conformal radiotherapy).The advancements in Linear Accelerator technology enable the production of the high-dose radiation in a short time. Thus, both the duration of sessions can be shortened and the high-dose radiation can be delivered to the tumor in a single session.In treatment of cancer, the radiotherapy can be used alone or combined with surgery or chemotherapy or applied before and after such treatments. Also, the radiotherapy is performed during the surgery for certain cancers and this method is called intraoperative radiotherapy. The purpose of the radiotherapy is to shrink the tumor before the surgery and eliminate possible microscopic cancer foci after the surgery. &nbsp;&nbsp;Also, it can be a definitive therapy when combined with chemotherapy. In case the definitive therapy is not possible for a particular cancer, the radiotherapy can be considered to improve quality of life (alleviation of pain, management and prevention of bleeding) (palliative therapy).Patient-specific Treatment PlanRadiotherapy uses various techniques, including but not limited IGRT, IMRT, conformal radiotherapy, stereotactic brain surgery and total body irradiation. These techniques depend on the Linear Accelerator of the device and each machine has unique superiorities.After the final diagnosis of cancer is made, it is investigated whether the patient is a good candidate for radiotherapy and radiation oncologist decides the best radiotherapy technique for the patient. Later, 3-dimensional images of the tumor are obtained through computerized tomography and the volume of the tumor is determined. &nbsp;&nbsp;The radiation oncologist determines the necessary dose for the tumor focus in cooperation with the physics engineer. This phase is called simulation and planning. &nbsp;&nbsp;The skin where x-ray beams will pass through is marked. Although indelible pens are used for this purpose, the patient should take care of these marks. In case the marks erases, the patient should never mark the skin again.Then, the decision is made regarding the number of sessions and days required to deliver the pre-determined total dose and necessary appointments are scheduled. Radiotherapy is performed five days a week. Weekends are spared, so the patient can rest and the tissues can regenerate.The patient presents to Radiation Oncology Clinic on the scheduled date. Radiotherapy plan is entirely reviewed before each session. The patient is transferred to the radiotherapy room and lies on the table of the Linear Accelerator. If required, thermoplastic masks and head frames are used to ensure immobility. &nbsp;&nbsp;The patient stays in the room for 15-30 minutes, but the radiation beams are delivered to the tumor for only a few minutes. Even though the radiation technician stays in a separate room during the radiotherapy, he/she can continuously communicate with the patient through a microphone and speaker.Although the machine works a little bit loud while the patient is positioned and the beams are delivered, the patient cannot hear or see the radiation. Moreover, radiotherapy does not cause pain.Although treatment is scheduled in sessions, the radiation oncologist evaluates response to treatment in every week and makes necessary modifications.In internal radiotherapy (or brachytherapy), the patient emits radiation for a while after the treatment, and the patient is, therefore, isolated from other people for a certain time. On the other hand, the patient does not emit radiation after external radiotherapy.Side effects of the radiotherapy are directly related to the target area of the radiotherapy. The radiation oncologist and nurse cooperate with other medical disciplines for management of these side effects, when needed. In addition, patients are informed about the potential side effects before and during the treatment. Hair Transplantation

Hair transplant is a surgical procedure. Although various methods are utilized in hair transplant, the commonly used techniques with proven efficiency are Follicular Unit Transplantation (FUT), Follicular Unit Extraction (FUE) and Direct Hair Implantation (DHI).

Hair Transplant Methods

Having a little longer history comparing to the other two methods, FUT method implies removing strips of healthy scalp from the nape region. Each of these strips called graft includes 1 to 4 hair follicles. These strips are sutured to the scalp where there is thinning of hair or baldness. &nbsp;&nbsp;These sutures are removed within 2 weeks on average. Although an insignificant scar remains due to sutures, it is not visible unless the hair is cut too short. &nbsp;

Follicular Unit Transplantation (FUT)

Solving the scar problem in Follicular Unit Transplantation, Follicular Unit Extraction (FUE) is the most commonly used hair transplantation technique.In FUE, the grafts that are called follicular unit and include 2 to 3 hair are removed from the nape region using very fine (0.5 mm) needles. FUE technique enables use of other hair in body when there is not enough hair in nape region and this provides an advantage. &nbsp;&nbsp;Since follicular units are harvested individually, suture is not necessary. As the procedure can be performed under local anesthesia or sedation, the patient does not suffer pain. The pain can be managed with simple painkillers after the procedure.<img src="https://res.cloudinary.com/dxqhh15dp/image/upload/v1672388910/Hair_Transplantation_70afc84c2b.webp" alt="Hair-Transplantation.webp">Hair is analyzed principally in all hair transplant techniques. The procedure is planned considering the anterior hair line and angle of follicle canals. The aim of this planning stage is to ensure a natural appearance of hair after transplant.In FUE method, hairs are cut very short before the procedure. However, Direct Hair Implantation (DHI) method ensures transplantation of hair without cutting the hair for suitable cases.After hair is cut short, follicular units are individually collected from the nape region using a micromotor. Those grafts are kept in a special solution until the transplantation stage. &nbsp;&nbsp;Next, the canals where these follicles will be placed are opened and follicular units are implanted. In the final stage, wound dressing is performed for the donor and recipient regions and the patient can be discharged. All of these procedures are completed in 6 to 8 hours.There is no difference between FUE and FUT techniques regarding natural appearance provided that they are performed accurately. &nbsp;However, FUE technique is more commonly employed due to its advantages.Mild swelling, bruising and numbness in scalp are expected symptoms following hair transplant. Most of them spontaneously disappear without an intervention. &nbsp;&nbsp;Your physician will inform you about the curst formations that will develop on scalp as a result of wound healing. Hairs are generally washed at the clinic for the first time after the procedure. At this stage, the patient is informed about how to wash the hair.Although hairs start to grow shortly after hair transplant, they absolutely shed generally within 1 to 2 months. This commonly seen condition is called shock hair loss and it is an expected condition. &nbsp; &nbsp;The lost hairs will re-grow within 3 to 4 months. However, there is a very high risk of permanent shock hair loss and loss of hair in the donor site if the procedure is performed by inexperienced individuals. &nbsp;

Follicular Unit Extraction (FUE)

The damage in donor site is minimized as the follicular units are harvested one by one. This technique does not cause scars as it does not require suturing. Most importantly, it allows treatment if the underlying cause of hair loss or baldness is a trauma like burn. &nbsp;&nbsp;As the damage in the scalp is minimized, recovery period is short and patients can resume their daily lives in a shorter time. &nbsp;

Advantages of FUE

Even if it is a simple procedure, hair transplant is essentially a surgery. Individuals who have heart disease, chronic diseases and who use blood thinners should necessarily consult to their physicians and hair transplant physician and inform them about their conditions.On the other hand, the procedure should absolutely be performed at hospital settings due to potential complications of minor surgeries, particularly infection.

Infertility is defined as the inability to achieve pregnancy despite regular and unprotected intercourse for one year.The unit that focuses on treatment of infertility is called the Assisted Reproductive Techniques Center. Male factors of infertility include low sperm count, absence of sperm in semen (azospermia), hormonal factors, varicocele and similar conditions. &nbsp;&nbsp;On the other hand, examples of the causative female factors include obstructed Fallopian tubes, hyperprolactinemia, endometriosis, anti-sperm antibodies, polycystic ovary syndrome, low egg reserve, genital infections and adherence of intra-abdominal organs.The first step to be taken for a couple, who presented with infertility, includes a comprehensive gynecologic exam, a comprehensive urologic exam, medical and family histories of man and woman, hormone analyses which may provide clues regarding infertility, sperm analysis, hysteroscopy, smear test and assessment of documents regarding previous trial(s) in the event that they have a history of infertility treatment. &nbsp;&nbsp;Egg reserves of the woman are assessed with hormone tests and ultrasonography on the third day of the menstrual cycle. The first step aims to identify the exact underlying cause of the infertility. The second step starts when it is clarified that the couple is a good candidate for in vitro fertilization.On the second stage, ovaries are stimulated with a medication to ensure maturation of follicles. Each healthy woman releases only one egg per month, but this medication aims to have ovaries release more than one egg. Follicles are monitored with ultrasound scan and another medication is administered to release the egg from the follicles after they reach a certain size. &nbsp;&nbsp;The last phase of this stage is oocyte pick-up (egg retrieval) under sedation or local anesthesia.The eggs retrieved from the woman are fertilized with sperms in laboratory, if there is no problem regarding motility, morphology or count of sperms. If any problem is detected in sperms, it is necessary to transfer sperm of the man into the egg with a very thin needle in order to ensure the fertilization. &nbsp;&nbsp;In both cases, resultant embryos are assessed by an embryologist and healthy embryos are selected. If there are more healthy embryos than needed, embryos are frozen and stored for future attempts.

Infertility

After eggs are fertilized with IVF or ICSI, pre-implantation genetic diagnosis is required to avoid transmission of any disease to the baby, if health and family histories of woman and/or man are notable for any genetic disease. &nbsp;&nbsp;After harvested embryos reach the blastocyst stage, healthy embryos are selected by the embryologist and only these embryos are transferred to the womb of the candidate mother. &nbsp;&nbsp;Pre-implantation genetic diagnosis is an important method which assists selection of the healthy embryo and decreases the risk of miscarriage.<img src="https://res.cloudinary.com/dxqhh15dp/image/upload/v1672400553/IVF_Treatment_Stages_Process_ac0639bce3.webp" alt="IVF-Treatment-Stages-Process.webp">On the third stage, the fertilized eggs grow for 2 to 3 days. Bone Marrow and Stem Cell Transplantation

Inability of a bone marrow to produce blood cells sufficiently or failure to produce blood cells due to any reason whatsoever will lead to numerous problems, including anemia, being prone to infections and bleeding diathesis. &nbsp;&nbsp;Blood cancers (leukemia, lymphoma), various genetic disorders and the diseases which involve the bone marrow may be the underlying cause of the dysfunction in the bone marrow. Why Is Bone Marrow Transplant Surgery Done? 

Bone marrow transplantation is classified into two types. First, it is classified according to the donor of the bone marrow or the stem cell. Autologous bone marrow transplantation implies that the bone marrow is obtained from the patient, while the bone marrow of a healthy donor is used in allogeneic bone marrow transplantation. In allogeneic transplantations, the donor can be the sibling (ideal donor), blood relatives or any unrelated person. In syngeneic transplantations, stem cells of the identical twin are used. &nbsp;&nbsp;The second classification is based on the tissue transplanted. Bone marrow transplantation refers transferring the bone marrow tissue to a patient, while stem cells are used in Peripheral Blood Stem Cell Transplantation. In the cord blood transplantations, stem cells are harvested from the placenta and the umbilical cord. &nbsp;&nbsp;Autologous Transplantation Drugs that are used in treatment of certain diseases may suppress the bone marrow tissue, leading to permanent damage. In such diseases, healthy bone marrow of the patient is harvested, frozen and stored before the treatment is started. The healthy bone marrow is transferred back to the patient after the disease is treated with high-dose medication(s).&nbsp;The risk of rejection and the possibility of stem cells to attach native body tissues are low, as the patient’s own bone marrow tissue is used. Autologous transplantation is, therefore, safer than the allogeneic transplant.Allogeneic Transplantation Use of bone marrow or peripheral blood stem cells obtained from the sibling of the patient, a blood relative or an unrelated person becomes necessary when the bone marrow of the patient loses its functions due to various reasons. Similar to autologous transplantation, high-dose medications are administered for treatment of the diseases before the allogeneic transplantation. &nbsp;&nbsp;The risk of rejection or Graft Versus Host Disease (native cells are attacked by donor cells) is lower in allogeneic transplantations, as other immune cells of the donor are transplanted to the recipient along with stem cells that form blood cells.

What Are Types of Bone Marrow Transplantation? 

A series of examination and procedures are required to assess overall health of the patient and to prepare the patient physically for transplant surgery after it is decided that disease of the patient can be treated with bone marrow transplant. These tests, which investigate if the recipient and donor meet certain criteria, may take several days to complete. &nbsp;&nbsp;A catheter is inserted into one of the major veins inside the patient’s body shortly before the transplant stage.Bone marrow can be suctioned out of the bone with a special needle in autologous transplantation cases. &nbsp;&nbsp;Another method is to administer a medication to the patient which increases the stem cell production in the bone marrow and ensures that the stem cells are released into blood. Next, stem cells inside the patient’s blood circulation are harvested with a procedure, called apheresis. &nbsp;&nbsp;Next, stem cells or bone marrow are frozen and stored to be used after treatment of the main disease. The first step of allogeneic bone marrow transplant is to find an appropriate donor for the patient. The stem cells obtained from peripheral blood or bone marrow of the donor are frozen and stored until the transplant surgery is performed. &nbsp;&nbsp;For both transplant groups, the cancer, condition which damages the bone marrow, is treated with high-dose medications before the bone marrow transplantation. Next, immune system of the patient is suppressed and the frozen blood cells are transferred to the patient via catheter. Stem cells migrate to the bone marrow of the patient following bone marrow transplant and divide there. &nbsp;&nbsp;Next, production of healthy blood cells begins. The immune system is suppressed for a period of time determined by the physician. Moreover, risks of infection, bleeding and anemia caused by low blood values are supervised and treated.&nbsp;The bone marrow starts functioning properly and the blood cell counts are restored to normal ranges within a few weeks under normal circumstances, but it may also take longer in case of any complication. Patients are transfused red blood cells or platelets to manage the conditions caused by low blood counts in this period. &nbsp;&nbsp;Otherwise, infections or other complications may prolong the hospital stay and cause negative effects on the success of the transplant.The transplant patients are informed in detail regarding precautions they are required to take for their lives after the transplant before they are discharged.
